The book by Matthew Syed. ‘Black box thinking – Marginal gains and the secrets of high performance’ is an interesting read. It highlights how the aviation industry has made significant gains due to its open approach to errors and failure contrasted with the healthcare sector and the criminal justice system which either consciously or sub-consciously can be in denial over errors made.
